Responsible for planning, developing, directing the vision, and evaluating strategy and activities for the System's Community Health Department and the development and oversight of programming in alignment with relevant community health improvement strategies within the Southwest Michigan area. Establishes and maintains partnerships with community organizations to address Community Health priorities, acquire grants, execute and evaluate funded programs and manage projects. This position is responsible for oversight and direction of Community Benefit Reporting, and the development of the Community Health Needs Assessment and corresponding implementation plans. Employees providing direct patient care must demonstrate competencies specific to the population served.
Master’s Degree in a healthcare related field or a Bachelor’s degree with equivalent experience 4 years of experience in a healthcare research environment writing grant proposals, manuscripts and other research documents
• Specialized knowledge of community health and/or public health theory and practices required • Excellent verbal and written communication skills required • Ability to plan, implement, and evaluate public health programs is required • Must have knowledge of current computing technologies and software; applications relevant to the position (e.g., Microsoft Windows, Outlook, Word, PowerPoint, Visio and Excel) • Strong interpersonal skills are required to develop and maintain effective and appropriate working relationships with consumers, colleagues, and representatives from community agencies and funding organizations • Must have considerable tact and communication skills in order to negotiate with, gain the acceptance of and/or change the thinking of others in difficult and complex situations • Must have the ability to facilitate meetings effectively and efficiently; experience in public speaking and presentation strongly preferred Work which produces levels of mental/visual fatigue which are typical of jobs that perform a wide variety of duties with frequent and significant uncontrollable deadlines. Work may include the operation of and full attention to a personal computer or CRT up to 40 percent of the time. The job produces some physical demands. Typical of jobs that include regular walking, standing, stooping, bending, sitting, and some lifting of light weight objects. • Plans and administers community health strategy and programing within the Bronson service region; establishes priorities, monitors and evaluates the effectiveness and efficiency of programs, develops and implements plans within the community to improve community health. • Develops collaborations with leaders within the community and Bronson as appropriate in the development of strategic goals and plans for the achievement of improved population health in Southwest Michigan. • Identifies and evaluates the need for new programs for prevention and control of priority community health needs particularly within vulnerable populations; recommends and manages community health department operations to implement new programs or modify existing programs • Manages the Community Health Needs Assessment process, conducts relevant research, develops an implementation plan, and ensures compliance with Affordable Care Act legislation • Supervises, trains, and provides leadership for community health team • Supervises, develops rotation programming, mentors, and provides leadership for Western Michigan School of Medicine students completing an Active Citizenship Rotation within the Bronson Community Health Department • Provides leadership and collaborates with applicable community advisory boards, committees, and task forces to identify gaps in community health services and to implement community wide programming; participates in and or reports to a variety of meetings, or related groups to communicate Bronson strategies within the community • Promotes Bronson Community Health strategies and programing within the community through interviews with television, radio, and newspaper media • Prepares and monitors funding and budget information for Community Health activities • Processes, compiles, researches, and analyzes complex community health data and other information for the evaluation of community health programing to include searching and summarizing reviews of clinical- and population health-related research literature. • Oversees and develops Certified Application Counselor and Navigator processes within Bronson, including training requirements, filing appropriate paperwork with state and federal agencies, and establishing and evaluating appropriate internal processes to ensure compliance • Directs the evaluation and maintenance of proper documentation for grants and billable services as they relate to the Community Health Department. • Researches grant program opportunities, maintains understanding of research and program grant needs within Bronson and the community with responsibility for overseeing all aspects of grant preparation for Bronson Community Health Department and other departments as needed. • Prepares written report and provide verbal updates of community health department activities to the Vice President of Community Health and the Community Health Board Committee • Compiles high quality written clinical and population health documents for funding applications and publication and searches and summarizes reviews of clinical- and population health-related research literature. • Establishes and maintains effective relationships with prospective grant-making organizations, collaborating organizations, and Bronson Research and Community Health partners. • Attends bidders’ conferences, RFP/RFA workshops, public hearings and any meetings pertaining to obtaining or maintaining funding. • Manages the process of supplying progress reports, as required by grant making organizations.
